What documents are needed for enrollment?

Any student entering school for the first time must present: (PCSS policy 6.203)

  1. A birth certificate or officially acceptable evidence of date of birth at the time of registration;
  2. Evidence of a medical examination of every student entering school for the first time;
  3. Evidence of state-required immunization;
  4. Proof of Residence (forms required – may include but not limited to: utility bills, lease, phone bill, property tax record, mortgage, etc..)
  5. Evidence of custody in certain cases.
